COVID-19 (novel coronavirus) Self-Monitoring Form
  
This self-monitoring form has been developed for two groups of people:
  
-All students and staff/faculty who traveled out of the country during Spring Break are being asked to complete this form in an effort to monitor their health and the health of the campus.
  
-Any student or staff/faculty member who is concerned that they may have been exposed to someone who has been infected by the COVID-19 virus.  
  
Answers to these questions will come directly to Lee University Health Clinic (LUHC) personnel and HIPAA regulations will be maintained.  Please ensure the phone number listed is your preferred method of contact.






Where do you live while at school?  If on campus, please specify the name of your dorm or apartment.
 
 


Have you been exposed to a known or suspected case of COVID-19?

Are you currently experiencing any of the following symptoms?  Please check all that apply:

Please continue to self-monitor, and if you develop any respiratory symptoms within the next 14 days, call the Health Clinic (423.614.8430) and ask to speak to a nurse for medical guidance.  Also, please be vigilant in washing your hands with soap and water, covering your mouth when you sneeze or cough, and keeping your hands off of your face.
  
If you currently have a fever, breathing problems, and/or an accompanying cough, please contact the Health Clinic by phone immediately at 423.614.8430.  If you are attempting to call after 5:00pm or on the weekends, we encourage you to seek medical assistance at the emergency room at Tennova Healthcare at 2305 Chambliss Avenue NW.
